2025–2026 Winter Physicals

Location: Kiski Area High School – Lower Cafeteria

All student-athletes must bring:

Completed PIAA Section 5 and Section 6 physical forms
Forms must be fully completed and signed by both the student and a parent/guardian
 Athletes without completed and signed forms will not be examined.

Additional Reminders:

• Physical forms (Sections 5 & 6) are available on the Kiski Area School District website under:
    Departments & Services → Athletics
  
   
• All athletes must also be registered on www.familyid.com
  
   

Impact Testing:
Kiski Area High School will administer the ImPACT Test for student-athletes participating in contact sports.
  Testing will take place at a later date—details to follow.
